Shipping Container Agreement

Shipping packages containing hazardous materials will, in most cases, require specific packaging for the safe transport of your materials. "Combination Packaging" consists of specific inner and outer containers, approved and tested to meet the transportation specifications required by Federal and International regulations. Whenever "combination packaging" is required to ship a hazardous material, it is your responsibility to provide the necessary packaging. Health and Safety is not responsible for providing the packaging, nor are we responsible for the shipping costs.

If your shipment requires approved combination packaging and you do not have the appropriate materials, please list a University account on the Transportation Profile Form that will be used to replace any packaging materials used in this shipment. Contact Health & Safety for estimated packaging costs.

By Clicking the "I Agree" button below, I agree that: (i) I have read and understand all of the terms and conditions set forth above; and (ii) that I am responsible for providing or replacing any shipping containers used by me or by my department.
