Verification Statements
A Verification Statement verifies that you have completed an
accredited or approved dietetics program.
What is the purpose of Verification Statements? Verification
of completion of dietetics programs is the method used by the Commission
on Accreditation for Dietetics Education (CADE) to ensure that academic
and supervised practice (also known as dietetic internship) requirements
for Commission on Dietetic Registration (CDR) eligibility for the Registration
Examination for Dietitians have been met. At various times in preparing
for professional membership or registration, an individual will be asked
to supply verification of both academic and supervised practice (internship)
qualifications. Therefore, it is the responsibility of the individual to
obtain the appropriate quantity of Verification Statements from their program
directors, and to safeguard them until the time they are to be used in
application processes.
Who completes and signs verification statements? The
Verification Statement forms(s) are to be signed and completed by the current
dietetics program director. The signature must be that of the program director
on record with CADE at ADA when the form is signed. The form should be
signed on or following the date of program completion. Statements that
are pre-dated or pre-issued are invalid. For students earning their baccalaureate
degree, verification statements will be issued when the student has completed
CADE-accredited/approved didactic program requirements and the registrar
has cleared the student for graduate, which indicates that all degree requirements
have been completed.
If, in the future, you need a verification statement
for ADA active membership status or registration eligibility and you do
not have a verification statement, contact the current program director
of the dietetics program that you graduated from to request one.
