This form is an order form for our LSSA T-shirt. There are options and prices specified for each on it. Please print the form off, fill it out, and turn it in to Mrs. Carol Reid in the Legal Studies office in 203 Odom Hall. You may also mail it in to the address stated on the form. If you are mailing your form please include a return mailing address and your contact information. Once we get all the forms in the T-shirt order will be placed and everyone will be notified when their shirts will come in. Thank you for your support and your interest in our organization!
