What should I do if my health status changes?
If you experience health status changes, such as pregnancy, immunosuppressant therapy, cancer, surgery, etc., you should take advantage of the free confidential consultation and recommendations from the Occupational Health Physician (OHP). Contact the IACUC office (915-7482 or askiacuc@research.olemiss.edu) for assistance.
Do I need to resubmit the Occupational Health Evaluation and OHSP Risk Inventory forms?
No. You should pick up your previously completed forms from the IACUC office and take them with you to the OHP.
What if my animal model changes?
If your animal model changes, you should (a) complete a new OHSP Risk Inventory form, (b) get your previously completed health forms from the IACUC office, and (c) take everything to the OHP.
