UNIVERSITY CABLE TELEVISION CONTRACT You have requested television service to your room for programming services not contained in the basic service available to all residence hall rooms. By signing this contract, you are agreeing to follow all rules and regulations contained in the contract. Any violation of this contract will be grounds for immediate termination of service and possible disciplinary action deemed necessary by the University. The rules and regulations are as follows:

• All television charges must be paid within 10 days of receipt of statement from the Bursar. Failure to pay all charges billed by the Bursar when they become due will result in your television services being terminated until the charges have been paid in full. A service fee of $25 will be charged to reconnect service if disconnected due to non-payment of your bill.
• You are responsible for all charges associated with the television services you want to receive. The bill for a television service package will be sent to the subscriber of services.
• Unauthorized reception of cable service illegal and is covered by the Communications Act of 1934 (SEC. 633. [47 U.S.C. 553] in which it states "(a)(1) No person shall intercept or receive or assist in intercepting or receiving any communications service offered over a cable system, unless specifically authorized to do so by a cable operator or as may otherwise be specifically authorized by law. Any person who willfully violates subsection (a)(1) shall be fined not more than $1,000 or imprisoned for not more than 6 months, or both." VIOLATIONS OF THIS CODE WILL BE ENFORCED AND HANDLED BY THE PROPER UNIVERSITY AUTHORITIES.
• There will be a $5.00 service fee charge for disconnection during the semester or transfer of services to another dorm room during the semester.

  •  The Works
• The University reserves the right to alter the rate structure and change programming services as approved by University officials.

NOTE: BEFORE YOUR TELEVISION WILL SHOW PROGRAMS IN THE REBELVISION AND PREMIUM SERVICES PACKAGE, YOU MUST PROGRAM IT TO RECEIVE THE CHANNELS ABOVE #13. PLEASE CONSULT YOUR OWNERS MANUAL FOR INSTRUCTIONS.
